Health and safety are a top priority at GDS.
GDS Summer Programs require that we receive a copy of your child’s most recent immunization records. You will find these forms during your registration process, but they are also provided below. We recognize that different states and doctors have different forms. We will accept different forms as long as they provide the necessary records. If you are a GDS family, you do not need to provide the DC Child Health Certificate. All other participants must complete this form.
If your child has an allergy that requires an Epi-Pen, we MUST have an action plan on file along with your child’s Epi-Pen. We also recognize that these plans differ from doctor to doctor. Please email us if you have any questions.
