Health and safety are a top priority at GDS.

While we do not have a nurse on campus during the day, we do have many staff members who are CPR and First Aid certified. These staff members are trained and prepared to administer basic CPR and First Aid if needed. We are also trained to give an Epi-Pen and an inhaler as needed. It is extremely important that you fill out all necessary health documents and provide us with information regarding any allergies so we can be prepared.

GDS Summer Programs require that the following health forms be completed. You will find these forms during your registration process, but they are also provided below. If you are a GDS family, you do not need to provide the DC Child Health Certificate. All other participants must complete this form.

Coming from abroad? You will need to provide documentation of a negative TB skin test.

All forms must be mailed to Auxiliary Programs at 4530 MacArthur Blvd Washington, D.C. 20007 or emailed to
